Abstract

Systemic Arterial Hypertension (SAH) is the most significant non-transmissible chronic disease; it reaches 25% of Brazilian adult population, affects more than 50% individuals over 60 years old, and may result in severe cardiovascular disease. CECOM created a treatment strategy to its hypertensive users. Approach SAH bearers in a collective way, motivating self-care and ahestion to treatment; stimulate dialogue and exchange of experiences and perceptions; contribute to the subject strengthening, so they can recognize they are able to cause changes in their lives; follow up and control the pathology. The SAH bearers are assessed by the team physician and nurse. Every meeting, every four months, it includes: BP measurement, information provided on demand, examination check, therapeutic and conduct assessment, according to CECOM's Internal SAH Protocol. We have 454 patients allocated to CUIDE-SE Program. The active search of absent patients ensure the necessary bond to the effective follow-up. CUIDE-SE aims to maintain the individuals on the recommended conducts and allocate their service, enabling assessment and follow-up by multidisciplinary team; sharing subjective disease aspects with the group, blood pressure monitoring and control, in addition to a support network creation. Since the installation of CUIDE-SE program, it was observed 40% decerase in the Emergency Service consultations of the practice by CID10 I.10 (Systemic Arterial Hypertension).
