Abstract

Unicamp&";s Hospital de Clínicas is a public, school hospital, 100% SUS, with refional reference and national reach. It has 410 beds and 17 at UER, with a monthly average of 2,000 surgeries and 31,000 ambulatory services. The Social Service acts in three main areas: Ambulatories, Wards and UER. It also has service in Gastrocentro, Integrated Nephorology Center (INC) and Hospital Dia. Other responsibility areas of this service are the Convocation Service, Reception Center to patient and driver and Death Service. We have specialized service through the following groups: Transplants, Oncology, Geriatrics, Pediatrics, Occupational Accident, Mental Health and other. However, the service duty is the place for access by the patient and family, since it constitutes a skilled listening space. It is a reference place, where the patient receives information, guidance, referrals and more complex cases. In this space, the hospital discharges from Wards and UER are performed, a moment when the professional establishes contacts and partnerships with the patient&";s reference services and one&";s needs to continue their medical treatment. The Ambulatory duty receives a large spontaneous demand due to the location at the hospital&";s entrance, and through the service referred by the health teams. The patient, in addition to one&";s health issue, brings questions full of social issues that cause difficulties on the care realization: transportation, medication, food, housing, etc., in addition to other risk and vulnerability variants. Therefore, the Duty is necessary and efficient for fulfilling an educative and assistance role.
